Patients can be referred to hospice palliative services by:
- Their GP (General Practitioner) Doctor
- Hospital Doctor
- Nurse Practitioner
- District Nurse
- Medical Specialist
- Self-referral
As a community organisation, anyone – be it the patient, family member or a health professional – should feel free to contact the hospice if they are concerned that needs of someone living with a life-limiting illness are not being met. It is important that before making this call you do have that patient’s permission.
